SURG 125 - Perioperative Case Management


3 Total Credits
This course provides students with a comprehensive understanding of the principles and application of Aseptic Technique in the operating room, encompassing the creation, maintenance, and safety considerations. Students will explore topics such as economy of motion, efficient supply utilization, surgeon preference cards, drains, and catheters. Additionally, the course covers essential aspects such as patient transfer, movement, handling, and positioning, surgical skin preparation and draping, case planning, intraoperative routines, and postoperative care of surgical wounds. PREREQUISITE(S): SURG 104 & 105

Course Outcomes
  • Recognize appropriate surgical attire. Employ principles involved in donning surgical attire. 
  • Describe and perform the physical preparation and care that the surgical patient may receive prior to the surgical procedure. Evaluate the items on the pre-operative patient check list. 
  • State the purpose of proper identification. Demonstrate the identification process for a surgical patient admitted to the surgical suite. 
  • Identify methods of patient transportation. Discuss the factors related to the family members and transportation of the patient. Demonstrate the principles of safe transportation. 
  • Analyze laboratory reports in relationship to patient diagnosis and intervention. Review the patient chart for completeness. 
  • Analyze the procedure for obtaining informed surgical consent. Analyze the legal concepts of obtaining informed surgical consent. 
  • Discuss methods of patient transfer. Identify equipment utilized for safe transfer of the surgical patient. Employ the principles of body mechanics when transferring the surgical patient. 
  • Analyze the use, components and aides utilized to achieve various surgical positions. Detail the sections and functions of the OR table. Perform basic positioning. 
  • List the indications for urinary catheterization. Discuss the basic considerations for urinary catheterization. List the supplies required to perform urinary catheterization. Demonstrate urinary catheterization. Discuss the principles of monitoring urine output. 
  • Compare and contrast different types of skin preparations. Compare and contrast different chemical agents used for skin preparation. Describe the steps and rationales for surgical skin preparation. 
  • Discuss the purposes and legal responsibilities of counts. Describe the techniques used to prevent foreign body retention. Discuss when counts should be performed. Describe the methods for counting. Demonstrate the procedure for counting instruments, sponges, sharps and other items on the field. 
  • Describe various types of draping material used in surgical procedures. Select the appropriate drapes for specific positions and surgical procedures. Demonstrate the aseptic principles of draping the patient, equipment, and furniture. 
  • Discuss methods of obtaining specimens. Discuss the types of specimen containers. Describe procedure for validating specimen with surgeon and circulator. Describe the procedure for specimen labeling and transfer to appropriate department. Discuss areas for specimen storage. Demonstrate the handling and preservation for specific types of specimens. 
  • Analyze the principles of hemostasis. Differentiate among various methods of hemostasis. Assess special techniques of hemostasis. Demonstrate surgical technologists’ role in hemostasis. 
  • Compare and contrast the types and characteristics of various catheters and drainage devices. Correlate the correct drainage device for each drain. Compare and contrast the conceptual differences between gravity and vacuum drainage. Prepare catheters and drains for intraoperative use. Prepare anchoring devices for drains. 
  • Analyze and assess the factors that influence the closure of each wound. Compare and contrast suture materials, suture sizing and suture coatings and analyze their significance. Demonstrate proper suture selection, preparation, handling and cutting techniques. Diagram and describe needle points and needle bodies and demonstrate the proper placement, handling, loading and disposal of surgical needles. Evaluate various applications of surgical stapling instruments and demonstrate proper assembly of stapling instrumentation. Compare and contrast reusable and disposable surgical stapling instruments and analyze the advantages and disadvantages of utilizing surgical staplers. Compare and contrast biological adhesives and synthetic adhesives. Analyze and evaluate various tissue repair and replacement materials. Discuss the specific applications of synthetic mesh. 
  • Evaluate the purposes of surgical dressings. Analyze their importance to postoperative wound care. Compare and contrast the most used types of surgical specialty dressings. Describe the importance of proper surgical dressing application techniques. Apply proper principles of sterile techniques and demonstrate the application of commonly used types of surgical and specialty dressings. 
  • Compare and contrast intentional, unintentional, and incidental/chronic wounds. Analyze the mechanisms of wound healing, the inflammatory process, and the healing process. Evaluate the classification of surgical wounds, analyze factors that influence healing, and devise a plan to prevent postoperative wound infections. Demonstrate basic wound care concepts and apply the principles of asepsis to the practice of sterile technique. 
  • Describe tissue replacement materials. Demonstrate knowledge of biological wound cover materials. 
  • Demonstrate methods utilized to analyze and plan for the needs of the surgical case. Select the instruments, supplies and equipment needed for a surgical procedure. Describe the function of the physical components of the operating room. Demonstrate the arrangement, care, handling and assembly of operating room furniture and equipment. Describe the use of instruments and supplies. Demonstrate techniques for preparing the sterile field. Explain the procedures for draping furniture and equipment. Describe placing and securing supplies and equipment for use on the sterile field. Explain methods for monitoring the sterile field. Apply the principles of asepsis to the practice of sterile technique. Assess and anticipate the needs of the surgical team. Integrate variations of case management according to the surgical procedure. Demonstrate postoperative case management duties in an organized manner. Apply standard precautions to the performance of perioperative case management activities.
