MAP 230 - Navigating Healthcare Systems with Patient Engagement

This course introduces students to the Care Model, formerly known as the Chronic Care Model, a mode of delivering health care that draws on clinical information systems, decision support, and self-management support to provide comprehensive care. This model introduces students to various health care systems, terminology and health insurance. Topics will focus on individual and family centered care issues, communication and health literacy, transitions and resource referral. Relevant case studies and applied learning strategies are used to build skills and knowledge.
Course Outcomes

  1. Demonstrate enrollment process. 
  2. Explain plan eligibility requirements.
  3. Educate patients regarding benefits and options.
  4. Demonstrate understanding of health care terminology.
  5. Describe and differentiate between health care systems.
  6. Identify the essentials of care coordination. 
  7. Identify deficiencies in the patient’s knowledge, skills, ability, and willingness to manage their own health and care. 
  8. Discuss interventions to increase patient activation and promote positive patient behavior. 
  9. Explain healthcare terminology in layperson language.
